Before you start
- Profile form template fields are Title, Code, Status, Description, and Fields as key, label, type, required, and options.
- Title is the staff-facing form name. Code is the stable identifier used by system flows and should not be renamed casually after use.
- Status controls lifecycle: Draft for building, Active for staff use, and Archived when the form should stop appearing.
- Fields are entered one per line in the format key|label|type|required|option1,option2.
- Use stable field keys without spaces. Use clear labels that gym staff can ask customers without explanation.
- Required should be true only for information that staff must capture before the workflow can continue.
- Options should be used for controlled answers such as goal, source, package interest, or risk category. Free-text fields should be used for notes that cannot be standardized.
Daily workflow
- Create or update the form in Draft, review fields with front desk and trainer users, then mark Active.
- After launch, check a real submission on a profile before relying on the form for operations.
- Archive old templates instead of deleting or changing historical meaning.
Watch out
- Do not collect sensitive medical or identity information unless the gym has a real operating reason and staff know how to use it.
